Download Entertainer Questionnaire (For comedians/comediennes, musical acts, or other entertainers)

Procedure:

1. Print out and complete the above questionnaire

2. Mail the questionnaire along with requested promotional materials to the attention of "New Artist/Speaker Submission"

3. Please allow a minimum of two to three weeks before calling

4. Motivational-Speakers.com does not return any promotional materials

5. It is understood and agreed to that by sending Motivational-Speakers.com promotional material you are giving Motivational-Speakers.com expressed permission to publicize and promote artist/speaker in any manner it deems necessary, including but not limited to brochures, direct mail pieces and placement on any website controlled by Motivational-Speakers.com. By sending promotional material I give permission for Motivational-Speakers.com to edit the material in any manner it deems necessary. In submitting copyrighted material to Motivational-Speakers.com to place on its website, artist/speaker warrants that all necessary permissions have been obtained, and agrees to hold harmless and indemnify Motivational-Speakers.com for all costs, expenses, attorney's fees and judgements resulting from any claim of copyright infringement.
